Terms and Conditions for Physiotherapy and Performance Services

1. Scope of Services
1.1 The services provided by Driven Athlete ("the Company") include physiotherapy, strength training, and performance services.
1.2 The Company will make every effort to provide professional and effective services tailored to the individual needs of the client.
2. Client Responsibilities
2.1 Clients must disclose any pre-existing health conditions, injuries, or medical concerns before commencing physiotherapy, strength training, or performance services.
2.2 Clients are responsible for providing accurate information regarding their medical history, medications, and any changes in their health status during the course of the services.
2.3 Clients are expected to actively engage in the program, follow the advice and instructions given by the physiotherapist, and participate in a manner that positively influences program efficacy.
3. Appointment Scheduling and Cancellation Policy
3.1 Appointments can be scheduled through Driven Performance's website or contact information.
3.2 Clients are required to provide a minimum of 24 hours' notice for appointment cancellations. Failure to do so may result in a cancellation fee.
3.3 Driven Athlete reserves the right to reschedule or cancel appointments due to unforeseen circumstances, and reasonable efforts will be made to notify clients in advance.
4. Fees and Payments
4.1 The fees for physiotherapy, strength training, and performance services are outlined in Driven Athlete's pricing structure, available on the website or provided upon request.
4.2 Payment is due at the time of service unless alternative arrangements have been made in advance.
4.3 Late payments may be subject to additional charges.
5. Confidentiality
5.1 All client information, including personal and health-related details, will be treated with strict confidentiality in accordance with applicable data protection laws.
5.2 Driven Athlete may use anonymized and aggregated data for internal research and improvement of services.
6. Liability and Insurance
6.1 Driven Athlete carries professional liability insurance, but clients are encouraged to have their own personal insurance coverage.
6.2 Driven Athlete is not liable for any injury, medical issue, illness, loss, or damage sustained by clients during or as a result of physiotherapy, strength training, or performance services including testing, except in cases of proven negligence.
7. Termination of Services
7.1 Driven Athlete reserves the right to terminate services if a client fails to comply with the terms and conditions outlined herein.
7.2 Clients have the right to terminate services with 30 days written notice.
8. No Guarantee of Program Efficacy
8.1 Clients acknowledge and understand that the efficacy of the physiotherapy, strength training, and performance services program is influenced by their active engagement and commitment, as well as other external influences outside the control of the Company.
8.2 Driven Athlete does not guarantee the success of the program in meeting the client's individual goals, and individual results may vary based on the client's level of engagement and other external factors beyond the Company's control.
9. Governing Law
9.1 These terms and conditions are governed by the laws of England and Wales.
9.2 Any disputes arising from these terms and conditions will be subject to the exclusive jurisdiction of the courts of England and Wales.
10. Amendments
10.1 Driven Athlete reserves the right to amend these terms and conditions at any time. Clients will be notified of any changes in advance.
By engaging in physiotherapy, strength training, and performance services with Driven Athlete, clients acknowledge and accept the above terms and conditions.
